Family and Medical Leave Act ( FMLA ) forms require a processing time of 14 business days, and an applicable $85 fee.

School, Sports Participation, and Camp Forms

Please allow up to 5 business days for the completion of all school, sports participation, and camp forms. A $30 fee will apply.

FMLA forms take 14 business days for completion. An $85 fee will apply.

Expedited processing and portal uploads are unavailable during the summer season ( June-September ), as this is our busiest season. Expedited processing of School, Sports Participation, and Camp Forms is available October through May for a fee of $40 per child and require a processing time of 1-2 business days.

Zelle, Cash or Major Credit Cards ( fee applies ) are accepted.
